Minimum Information Required to create Banner ID

A new record must include the following information as part of the set up. A Banner ID cannot be assigned without the information below. However, in extreme cases we can create records that meet the minimum person record requirements by only including the information below in bold with an asterisk (only one of the three contact types (items 6 – 8) is absolutely required)

  1. Last Name* (full legal name)
  2. First Name*
  3. Middle Name
  4. Other names that may appear on records
  5. Birth date*
  6. Current Mailing Address*
  7. Home Phone*
  8. Current Email Address*
  9. Gender
